Why Some Women Prefer to Have it Smaller

Although large breasts are often associated with being sexy and attractive, excessively large breasts may actually do more harm than good. It could also be extremely difficult for these women to find clothing that can accommodate their larger than normal breasts.

In addition to the physical issues, being large-breasted may also lead to feelings of insecurity or embarrassment. Many of our patients report that they are tired of being stared at because of the appearance of their breasts. Such feelings could lead to reduced self-esteem and depression.

  • You may have read or heard about the things that need to be considered before getting breast implants. FIrst, you need to think about implant type and size, implant placement and surgical incisions. And there’s the matter on choosing the right plastic surgeon where you have to consider experience, credentials, communications skills, and your level of comfort during the personal consultation.

Yet before you go all through of the aforementioned, there is one crucial component to consider if you’re seriously contemplating about having breast implants — why are you having them in the first place?

    Examining Your Reasons and Expectations

The decision to have breast augmentation should not be taken lightly. By and large, you should not choose to have the procedure unless you have carefully figured out your motivation in wanting to have your breasts enhanced with implants. Like all forms of plastic surgery, there is no guarantee that breast implants can lead you towards the road to perfection. Keep in mind that plastic surgery will not help you get your ex back or rekindle a relationship gone stale. On a whole, breast augmentation is all about improvement rather than creating perfection.

Moreover, you might also want to assess your expectations of the procedure. It is unreasonable to expect that you’ll wake up with the perfect pair of breasts immediately after surgery. The truth is you’ll wake up with significant pain, swelling, and bruising hours following surgery. It may take months before the outcomes you want to see will materialize. Lastly, it is important to note that each person is unique — from genetics to lifestyle habits — and not everyone will achieve results at the same rate.

    Planning for Your Breast Augmentation

Your first visit at our practice is an excellent opportunity for you to communicate your concerns and expectations in a breast augmentation procedure. Call us at 415.398.7778 or fill out this contact form today to schedule an appointment. We look forward to your visit!

  • There are tons of information online promoting the benefits of breastfeeding and how it’s far more superior than formula feeding. By and large, it doesn’t only benefit the newborn but the new mother as well. Here at our practice, it’s not uncommon for women to ask about the role of breast implants in breastfeeding. What if they would get pregnant following surgery? What factors should be considered while making the breast augmentation decision when you have future plans of breastfeeding? We answer these questions and more below!

Breastfeeding After Breast Augmentation is Possible

Although many women who consider breast enhancement procedures may have decided not to give birth anymore, there are some who later change their mind while others find themselves with a surprise pregnancy. In both circumstances, breastfeeding is actually possible following breast implants. However, it should be noted that a significant reduction in milk supply may result when ducts or nerves are severed during surgery. Thus, the type, location, and extent of incision plays a significant role in determining whether or not milk production will be affected after having breast implants.

    The Incision Decision

Generally, there are four incision types that are currently used in breast augmentation surgery: the inframammary incision (in the fold underneath the breast), periareolar (just around the areola), transaxillary (around the armpit), and TUBA (trans-umbilical breast augmentation).

The inframammary incision is the most commonly used amongst the four types. It also offers the least possibility of severing the milk ducts which may hinder future plans of breastfeeding. On the other hand, the periareolar incision (particularly in the lower outer quadrant around the perimeter of the areola) offers the greatest risk of reduced nerve response to the areola and nipple which in turn may result to reduced milk ejection.

  • Every so often, patients are eager and overly excited with their plastic surgery outcomes. Although recovery from plastic surgery is influenced by various factors such as the type of procedure and the general state of their health, follow-up visits following surgery also play a crucial role in how the results will turn out. Yet, there are still patients who may find it unnecessary when Dr. Mosser instructs them to come in at our San Francisco plastic surgery practice for follow-up visits between 6 months to a year.

A Week or Two Following Surgery

During personal consultations before their scheduled surgery, we inform patients that the first to second week following surgery is critical. Dr. Mosser will look for signs of infection on the incision site and also check the status of surgical drains (if they were inserted such as in a tummy tuck). The degree of bruising or swelling will also be assessed around this time.

Measures will be taken if the incision is not healing well or if signs of infection are present. Pain medications and/or antibiotics may also be modified during the first follow-up visit. By and large, the goal of your follow-up visit during the first to second week post-surgery is to avoid delayed healing as much as possible.

Three to Six Weeks Following Surgery

Generally, bruising and swelling are the main focus of assessment during follow-up visits 3 to 6 weeks following surgery. Cold compresses and use of compression garments may be encouraged around this time. Moreover, signs of infection and possible scarring will also be thoroughly monitored.

    Six Weeks and Beyond

The outcomes of a majority of surgical cosmetic procedures may be predicted once the six-week mark has been reached. At around this time, Dr. Mosser will be able to figure out whether or not your cosmetic goals will be fully met. Interventions  such as revision surgery to help correct certain problems with recovery and healing will also be discussed during this period of your follow-up visits. Procedures to enhance results may be also recommended.

It may be frustrating for many to adhere to follow-up visits as instructed. However, it should be noted that healing after plastic surgery does not only involve a short time frame post-procedure. It takes a significant amount of time for the body to adjust and patience is, without a doubt, a virtue post-surgery.

  • image-forehead The presence of excess skin, bulging fat, and increasingly loose muscles in and near the eye area may give way to an older, tired, or perpetually upset appearance. The appearance of wrinkles, folds, and fine lines have always been associated with genetics, aging, and lifestyle factors such as smoking, nutrition, and sun exposure. However, a recent study in Netherlands just revealed that there are actually specific risk factors that may influence one’s likelihood of having sagging eyelids.

Here’s an excerpt of the study’s results:

Among the group of Dutch Europeans, 17.8 percent had moderate to severe sagging eyelids. Risk factors for sagging eyelids included age, being male, having lighter skin color and a higher BMI. Current smoking also may have some association. Among the twin pairs, heritability of sagging eyelids was estimated at about 61 percent.

    Sagging Eyelid Correction Surgery

The American Society for Aesthetic Plastic Surgery revealed that eyelid surgery was the third most frequently performed procedure last year, up by 5.4 percent from 2012. The procedure, also known as blepharoplasty, reduces the appearance of the following:

    • puffiness in the upper eyelids
    • sagging or loose skin in the upper eyelids which may impair vision
    • droopiness of the lower eyelids
    • under eye bags

Blepharoplasty may be done separately or in combination on the upper and lower eyelids, depending on the patient’s cosmetic needs and preferences. In some cases, a brow lift may also be done in combination with eyelid surgery to improve the appearance of the brows and restore their naturally elevated position. Non-surgical alternatives such as Botox and dermal fillers may also be done as a finishing touch to eyelid surgery.

    Can anyone have eyelid surgery?

Not everyone can have eyelid surgery. Generally, you are an ideal candidate for eyelid surgery if you are in excellent physical health and has realistic expectations of the procedure. In addition, you should be free (or well-controlled) of the following conditions:

    • high blood pressure
    • any type of cardiovascular disease
    • glaucoma
    • diabetes
    • thyroid problems such as Graves’ disease and hypothyroidism
    • dry eye syndrome

Furthermore, your existing facial anatomy such as bone structure may also influence your candidacy for sagging eyelid correction surgery.

  • image-botox Poet Kahlil Gibran may have once said thatbeauty is not in the face; beauty is a light in the heart.” However, no matter how we look at it, the face will always be one of the determinants of beauty. In fact, several researchers have been made to define and measure physical attractiveness.

In this 2009 study for example,  the researchers defined the golden ratio of female facial beauty. It turns out that beauty is not exactly in the eyes of the beholder but rather when the vertical distance between their eyes and the mouth was approximately 36 percent of the face’s length, and the horizontal distance between their eyes was approximately 46 percent of the face’s width.”

You don’t have to go to great lengths in achieving the so-called golden ratio in order to appear naturally youthful. Here at our San Francisco cosmetic surgery practice, a face lift procedure can help reduce the appearance of wrinkles, folds, and sagging jowls and create a more youthful appearance. If you’ve been considering a face lift procedure for a while, it pays to carefully examine the details of surgery as well as contemplate on  whether or not it is the procedure you’re looking for.

We answer the most common face lift questions that might help you make an informed decision below!

Am I too old to have a face lift?

On a whole, age is not a huge factor in influencing your candidacy for a face lift procedure. Instead, the specific determinants of whether or not you are an ideal candidate for a face lift include the severity of your wrinkles and facial sagging, skin elasticity, and the state of your general health. By and large, smokers are advised to quit smoking months before and after surgery as smoking can significantly delay healing and increase one’s risk of complications.

What if I don’t want to have a complete face lift procedure?

A traditional face lift procedure has 3 basic components: removal of fat, tightening of slackened facial muscles, and removal of excess facial skin. If you want to improve only a portion of the face or only reduce one aspect of facial aging (like removal of excess fat), a limited incision lift or a mid-face lift might be an alternative to the traditional face lift.

In addition, there are also facial rejuvenation procedures that may be done in conjunction with a face lift such as a brow lift, eyelid lift, and injectables.

How long will the results of a face lift last?

Generally, you can expect the results to last for a decade or so. However, this is just an estimation as there are certain factors that can influence how long the results will last such as your lifestyle (smoking habits, anyone), sun exposure, skin type, genetics, and overall health.

How do I find a good face lift surgeon?

A successful surgical outcome in a face lift procedure is largely influenced by your choice of plastic surgeon. First of all, it pays to find someone who has solid experience in performing the procedure. You can do this by personally asking for referrals from friends as well as checking the surgeon’s credentials. In addition, the surgeon should be able to answer all of your questions concerning the procedure. Moreover, you should be totally comfortable with the surgeon and the rest of his or her team during the consultation. Lastly, a good plastic surgeon should not force you to undergo a procedure unless you have made an informed decision.

    Is it safe?

All types of cosmetic surgery, whether invasive or minimally invasive,  have its own set of possible risks and complications. Bleeding, infection, and delayed healing are the most common risks associated with a face lift procedure. However, these complications can be easily prevented by sufficient preparation on your end and by choosing a highly experienced face lift surgeon who is adept in the procedure’s surgical technique and has excellent aesthetic sense.

  • Facial Rejuvenation You are not alone, you know that perfect triangular face your mom always said you had, and age has flipped that over for you. When you chin and neck appear larger than the rest of your face, it’s time to consider what choices are out there.

Research the options available, you won’t need all of them but it will help you pick the ones that you think will help. Then, get a consultation and don’t act surprised when they recommend something entirely different. Their experience gives them the edge, they see how it all turns out, so let yourself to be persuaded.

    The Choices

Fillers and line erasures are all the rage, surgery if any is minor and the results last a fairly long time. The bruising and down time is shorter than ever too. The type of filler you need may depend on the place its put. Be sure to ask the pros and cons of each suggestion, and then make an informed decision at the end.
You can get your eyebrows and eyelids redone, your chin, cheeks replaced, and your nose honed to a fine sculpture. You can even get those elephant ears reshaped and reduced and your forehead shortened. Heck, you might need to put on a nametag so you’ll know who you’re looking at in the mirror.

    Cheeks or No Cheeks

While one line is forming to have cheeks implanted, another is to have theirs reduced.
Does it ever fail, we all see ourselves differently than others do. Let’s face it – we have had more time to pick out the worst points and they seem to be getting larger.
In the end, your goals have to be reasonable and tolerable physically and financially.
You don’t want to look great and then be too sick to strut your stuff, so be cautious.

  • Mommy Makeover They have combined specific procedures to make mommy appear like she was before the blessed event of pregnancy. It lifts your breast up, and if needed, you can have their size altered. Your abdomen can be slimmed and tightened and before you know it, your girlish figure is back intact.
Some women are so depressed about the changes in their figures they wish they had opted for adoption. This will allow you to have both, the baby and the body.

Is Your Butt Dragging you Down?
Some women have bad luck or none at all. No breasts, no butt and just flab in between, you don’t have to live with that anymore. Your butt can be a proud accessory now, a source of compliments and well-fitting clothes, and your breasts will allow you to wear dresses requiring cleavage -when put together you will be a knock out.

    Enough is Enough
If you are too well- endowed, have your bust reduced to balance your profile. Breasts that are too large – can cause severe back and shoulder pain, causing people to slouch, which exacerbates the problem even further. Moreover, I don’t care how much support a bra provides, the weight is still an issue. It’s like carrying around a 20-pound barbell on your back, it’s exhausting.

You can have it done by liposuction, which only requires a local anesthetic. If you need breast reduction surgery that takes a longer recovery period and general anesthesia, but the result is very natural looking.

The best way to decide is to go and talk to the plastic surgeon. They can tell you what method will work best for you and why. They can also tell you all the specifics of do’s and don’ts while you are healing.

  • Facelift The hallmark attributes of youthful skin is similar across cultures and ethnicities: firm, elastic, smooth, and free of defects or blemishes. However, the human body changes over time and the skin is no exception. As the largest organ in the body, it undergoes a tremendous number of changes from infancy to childhood to adolescence to adulthood.

Every so often, loose, sagging skin is one of the noticeable changes that prompt men and women to seek Dr. Mosser’s expertise in providing cosmetic solutions. Read on below to learn more about ways to reduce and delay the appearance of sagging skin resulting from aging.

Factors with Major Influence on Sagging Skin

Generally, factors that influence the appearance of sagging skin include the following:

    • Effects of gravity – This refers to the natural downward pull as we walk, sit, and move in an upward position
    • Prolonged and consistent sun exposure
    • Gradual loss of collagen and elastin in the skin – Both are types of protein that naturally occur in the body and are responsible for the taut appearance of youthful skin. However, as one ages, the collagen and elastin in the skin tend to dwindle. Collagen loss is partly responsible in the appearance of crow’s feet (wrinkles in the outermost corners of the eyes) and the progression of jowls along the jawline
    • Dramatic weight loss – This could be a result to a change in lifestyle (diet and exercise), recovery from a long-term illness, or immediately after childbirth when the skin loses elasticity as it is stretched. Excess, loose skin is often noticeable in the arms, stomach, hips, and thighs. In addition, stretch marks may also appear as the skin is unable to adhere to the new body contour
    • Influence of hormones such as in the case of menopause when production of estrogen has been depleted.
    • Prolonged and consistent sun exposure
    • Substantial loss of fat tissue in the subcutaneous layer of the skin
    • Shrinking bones which result to lack of support for the skin

Other factors that may also influence the development of sagging skin include chronic lack of sleep, lack of exercise, poor diet, smoking, and alcohol.

Possible Cosmetic Treatments for Excess Skin

Face and neck – If you choose the surgical route, a face lift and/or a neck lift can help remove excess skin in the area. In addition, a brow lift or an eyelid lift can help remove loose, sagging skin on the upper third of the face. The aforementioned surgical procedures remove excess skin while the remaining skin is pulled into place for a more tightened appearance.

Non-surgical alternatives include the use of dermal fillers that primarily add volume to the skin. These injectables can be injected near or around the eyes, nose, lips, and neckline.

Body – Surgical cosmetic procedures are done in a similar fashion as the aforementioned surgical techniques for the face and neck. These procedures may include a tummy tuck, buttock enhancement surgery via implants or fat transfer, or a lower body lift.

  • Plastic Surgery Planning for plastic surgery involves a host of factors ranging from an individual’s cosmetic needs and preferences, lifestyle habits, and the state of his or her overall health and well-being. During initial consultations here at our San Francisco plastic surgery practice, it’s not uncommon for us to encounter patients who are keen to undergo surgery yet the presence of certain health condition or disease may further put their health at risk following the procedure. Hypertension is one of these pre-surgery health issues.

    Understanding Hypertension

Generally, blood pressure refers to the measurement of force exerted against the arteries (the blood vessels that carry oxygenated blood from the heart to the areas of the body). Blood pressure is at its highest when your heart pumps blood (systolic pressure) and it is at its lowest when your heart is at rest between beats (diastolic pressure). In a blood pressure reading, the systolic pressure comes before the diastolic pressure.

Generally, a reading of 120/80 mmHg is considered normal while a reading beyond 140/90 mmHg can already be classified as high blood pressure. The development of hypertension is caused by a variety of factors and may even be a result of another existing condition. By and large, the older you are, the greater the risk of high blood pressure as the arteries become stiffer over time. Other factors that can influence the occurrence of hypertension include but are not limited to the following:

    • hormone levels, particularly in women
    • use of certain medications and supplements
    • lifestyle habits such as smoking and alcohol consumption
    • excess body weight
    • family history of hypertension
    • stress and anxiety levels
    • your body’s ability to retain water and salt which may be influenced by one’s diet
    • in some cases, hypertension may be a result of existing conditions such as thyroid problems, disorders of the adrenal glands, pregnancy, diabetes, and chronic kidney disease

Plastic Surgery Risks Associated with High Blood Pressure

On a whole, having high blood pressure levels signify that your heart and blood vessels are experiencing too much strain. Having surgery when you’re hypertensive is like adding insult to injury to the body. The following risks and complications may be possible during and after surgery:

    • Bleeding during and after surgery, particularly if the patient is taking blood thinning medications prior to surgery
    • Hematoma or swelling filled with blood due to a breakage in a portion of the blood vessel walls
    • Heart or circulatory system problems

Can hypertension stop someone from having plastic surgery?

Yes, that is if you have uncontrolled hypertension. Here at our practice, we strongly recommend that a patient should work closely with their general doctor in putting their high blood pressure under control through medications and lifestyle revision. We can actually work with your primary care physician to make sure that you meet the criteria for undergoing surgery safely.

Take the results of this survey among plastic surgeons as an example. Here’s an excerpt from the conclusion (perioperative care translates to care before surgery):

Proper perioperative care of the facelift patient is paramount in the reduction of hematoma rates. Preexisting hypertension correlates with a higher hematoma rate, though this study also suggests that normotensive anesthesia, as well as strict postoperative blood pressure control did contribute to a reduction in hematoma rate.
